To register please complete and provide the following information:

Please complete the registration / commitment form.

Please complete the credit card authorization form (if applicable).

Please complete the medical waiver form (please have notarized).

Please provide a wallet size photo of the player to use for the player card.

Please provide a copy of the player’s birth certificate (N/A for current Fever Academy players).
