Manager Information

Team Application:

All teams mya register online. Payment may be made using a credit card during the online registration process or select a pay by check/later option.

Housing: 
 

  • To make the hotel booking process easy and organize please contact Dawn Potter, dawnp@plexsports.com, to make hotel reservations.
  • This is a play and stay event. All teams must go through hotel coodinator for bookings

Rosters and Check-in: 

All teams may check-in Firday night before the event or 1 hour prior to their first game at the Headquarters Tent. there

 

Call teams must provide:

Current/valid USSF Player Passes (US Club Soccer or USYSA) required for all players to verify against roster.
Approved State Association or US Club Roster
Medical Release forms for ALL players (you may use your own forms).

Application to Travel- "Teams Traveling from Outside Indiana Only":
USYSA form, approved by your state association. 

US Club teams do not need travel paperwork. 

  • If you have not received your player cards you must bring a copy of each player’s birth certificate and a picture which allows our staff to confirm with the state roster. 

  • Guest Player Instructions:
    You may add guest players at anytime up to 1 hour prior to your first game.
  • All teams may have up to 4 guest players. All players can only play on one team per tournament
